  6. If I'm not mistaken, the objection! icon has existed for a while and was incorporated into the AA mod, if it didn't inspire said mod in the first place.

    The "Paved With Good Intentions" achievement is related to the saying: "The road to hell is paved with good intentions".

    "And therefore, a witch!" comes from this classic:

    "Doctor Strangelove" is from "Doctor Strangelove or How I learned to stop worrying and love the bomb"
    "Faust" is from "Dr. Faustus", adapted multiple times (my favourite is Goethe's but that my be because I am German ;))
    "Simon Belmont" is a reference to a Castlevania protagonist.

    Now for older ones which I have not yet checked if they are already explained:
    "On her Majesty's steampunk service" is a reference to "On her majesty's secret service", a James Bond movie.
    Someone "Playing for both teams" is bisexual.
    "You know what time it is." You don't? Stop, Hammertime!: (Skip to 1:55)

    "Dead Man's Chest" is the title of the second Pirates of the Caribbean movie
    "I Didn't Ask For This" is a reference to the Deus Ex games, and a line that can (as far as I remember) be said in Deus Ex: Human Revolution (The Human Revolution wiki page also shows where the achievement icon came from ;))
    "Mister Smith" is first a pun, and secondly (WMG here) a reference to Agent Smith from The Matrix.
    "That Which Is Seen Cannot Be Unseen" is a meme. Meaning obvious.
    "It's Alive!" and "...And Gross!" are references to Frankenstein's Monster.
    "Turtle Power!" is a reference to the Teenage Mutant Ninja Turtles.
    "It Has A Nob On The End" is a reference to the Discworld song "A wizard's staff has a nob on the end."

    These are ones I could think of and I was not sure they were explained yet.

    Having a nob on the end of the staff would be hilarious. And it would be Orky as well.

    But they make texts so funny...
    Also, I knew what nobs are.
    I rather like WH40k, but have not the money to get an army myself :(

    The "A Mirror Darkly Shield," is a reference to the 2006 film, A Scanner Darkly. I was pretty surprised when I saw that. I'm a big fan of the movie.
