I've had this happen on a few characters, I get a quest to find an item and it's no where to be found. I've cleared the entire floor and went over it more then a few times making sure I'm not missing a room or corner. This takes quite a long time to do and I am almost positive this is a glitch since its happened a few times but I have not seen anything about this. I am wondering if anyone else has had this problem?

I think it's happening to me in my current game right now. I've completed every other quest on all the levels except for one on level 2, but no matter where I look I can't find it.

If it helps the one I'm playing right now its on level 2 as well and the 3 times before that were all on level 2 maybe its a glitch with the quest/floor?

Some items appear in parts of the map you've already been too, and they move when you go up and down floors too. From what i've seen anyway, i never leave a floor and always go check every room once or twice and usually find them in places where i've been hundreds of times previously.

Usually I don't bother to comb the level for quest stuff. When I'm not afraid of bosses, I pick up all quests and then, if I find the required whatever, good, if not, well yeah whatever รด_o

The problem is, Quest Items don't actually exist until you accept the quest from the Shrine of Inconsequentia. Once you click "Accept", the item is randomly spawned somewhere on the map.

The second problem is that sometimes, "somewhere on the map" is behind a wall, or under a doodad, or half-hidden behind a tall doodad, or the like, and when that happens, you can walk past it 3 times and not notice it.

So, while completing every Inconsequentia quest the game gives you is generally a good idea, sometimes you just have to assume the game screwed you over and just move on.

In fact that's my basic assumption. With picking all quests I meant "picking all quests", not their items, that is: praying to Inconsequentia.
