Dead, but not actually?

(First post, so sorry if things aren't quite right)

Was playing the other day, almost died. Saved right beforehand (Not playing with PermaDeath). After saving, I died immediately, then after loading, was still dead. I still have 4 hp, and I took a screenshot.

 

...heart attack?

 

Perhaps nicholas forgot to write the cause of death in his note.

Must be, considering. It actually kills me there too, after every reload. Not just the message, if I was unclear to begin with.

Disappointing, but not much I can do about it I guess.
