I've had this happen a couple of times now in those rooms with small 'islands' and multiple levers that teleport you around them in a circle. The lever will teleport you to a set square on the next island, but occasionally this square also ends up occupied by a random obstacle (eg. candelabra) or the next lever itself. In one case I also ended up teleported into the same square as an enemy. It's possible to just step back into an open tile, but it feels a little ugly.
