The belt bug is known, as for the bookshelf one it happens when a recipe is used by the n-Dimensional Lathe and your mouse hovers over the icon.
Sorry for not answering, it's an ancient bug, on the list.
L0 is random, it can have monsters from any floors in it.
If anything this would be useful to make pets scale.

To be fair, that folder is misleading :) it gets created automatically so one would assume they go there.

I don't know the answer to your pinning question since I've never used pinning in Win7 BUT did you know you can run DoD without steam? just run...
The normal one, levelup again, pick the same skill and see if it crashes.

Since your save is still there, can you verify whether it crashes again, or it was just a random crash? If so, please upload it, it will help a lot.
