Maybe Avenging Executioner is too much overpowered in the first floor level even without taking skillpoints into it: it is already a damage dealer - crowd controll skill, and with a 5:dmg_slashing: you can kill basically anything on the first floor, even if RNG is againt you.
Beside that, no other problem founded while running this, and it adds so much in the game it's basically fantastic standalone.

I think that your mod should implement the droppage of
Code:
-

<price amount="250" />
<mushroom />
<casts spell="DoT Rage" />

-

<price amount="800" />
<mushroom />
<casts spell="Antimagic Buff" />

-

<price amount="800" />
<mushroom />
<casts spell="Salty Bawl" />

for Fungal Art skill, like adding those effects for drop of 1st and 2nd level, or something like that?
Those mushrooms are awesome, but they sometimes feel like they are outside of real game, since they give you so little buffs and they don't drop regulary... it could feel more integrated in the game if you could add them to FA skill.

Is Pious Exhalation supposed to send you to a random location?
